The terms governing your use of DoseAdvisor and GlucoWorks services.
By accessing or using any website, application, or service operated by GlucoWorks LLC ("GlucoWorks," "we," "us," or "our"), you agree to be bound by these Terms of Service ("Terms"). If you do not agree to these Terms, do not use our services.
GlucoWorks develops and operates diabetes management software products and services, including CGMInsight™, Insulin DosePro, Insulize, and DoseAdvisor. These services include:

You must be at least 18 years of age (or the age of majority in your jurisdiction) to use our services. By using our services, you represent that you meet this eligibility requirement. Healthcare provider accounts are restricted to licensed healthcare professionals.
Certain services require account registration. You agree to provide accurate, current, and complete information during registration and to keep your account information updated. You are responsible for maintaining the confidentiality of your account credentials and for all activities under your account.
You may use our services only for their intended purposes and in compliance with these Terms, applicable laws, and any additional product-specific terms. DoseAdvisor is intended for use as a clinical decision support tool and should be used in conjunction with, not as a replacement for, professional medical judgment.
DoseAdvisor is currently authorized for use only within the context of an approved clinical investigation. Access is restricted to authorized clinical trial participants and their supervising clinicians. Any use of DoseAdvisor outside the scope of an approved clinical trial protocol is expressly prohibited and constitutes a violation of these Terms. Off-label or unauthorized clinical deployment prior to applicable regulatory clearance is not permitted.

All content, software, trademarks (including GlucoWorks, CGMInsight™, and DoseAdvisor), and other intellectual property on our services are owned by or licensed to GlucoWorks. You may not copy, modify, distribute, or create derivative works without our prior written consent.
GlucoWorks products, including DoseAdvisor, are intended to support — not replace — the clinical judgment of licensed healthcare professionals. DoseAdvisor provides insulin dose calculations based on parameters prescribed by a healthcare provider or configured by the user, and should not be considered medical advice. Always consult your healthcare provider before making changes to your diabetes management regimen. In case of a medical emergency, call your local emergency services immediately.
DoseAdvisor has not been cleared or approved by the U.S. Food and Drug Administration (FDA) as a medical device. GlucoWorks is currently pursuing designation under the FDA's Clinical Decision Support (CDS) guidance pathway. Until such designation or clearance is obtained, DoseAdvisor is available only for use within the scope of authorized clinical investigations. No representation is made that DoseAdvisor has received regulatory clearance for general commercial distribution or unsupervised clinical use.
TO THE MAXIMUM EXTENT PERMITTED BY LAW, GLUCOWORKS SHALL NOT BE LIABLE FOR ANY INDIRECT, INCIDENTAL, SPECIAL, CONSEQUENTIAL, OR PUNITIVE DAMAGES ARISING OUT OF OR RELATED TO YOUR USE OF OUR SERVICES. OUR TOTAL LIABILITY SHALL NOT EXCEED THE AMOUNT YOU PAID US IN THE TWELVE (12) MONTHS PRECEDING THE EVENT GIVING RISE TO LIABILITY, OR ONE HUNDRED DOLLARS ($100), WHICHEVER IS GREATER.
You agree to indemnify and hold harmless GlucoWorks, its officers, directors, employees, and agents from any claims, losses, damages, liabilities, and expenses (including attorneys' fees) arising out of or related to your use of our services or violation of these Terms.
We may suspend or terminate your access to our services at any time, with or without cause and with or without notice. Upon termination, your right to use our services will immediately cease. Sections that by their nature should survive termination shall survive.
These Terms shall be governed by and construed in accordance with the laws of the State of Wyoming, without regard to conflict of laws principles.
Any dispute arising out of or relating to these Terms shall first be addressed through good-faith negotiation. If negotiation fails, disputes shall be resolved through binding arbitration administered in accordance with the rules of the American Arbitration Association, conducted in Wyoming.
We may modify these Terms at any time. We will post updated Terms on this page with a revised effective date. Material changes will be communicated through notice on our Website. Your continued use of our services after changes are posted constitutes acceptance of the updated Terms.
If any provision of these Terms is found to be unenforceable, the remaining provisions shall continue in full force and effect.
These Terms, together with our Privacy Policy, Consumer Health Data Privacy Policy, and any product-specific terms or policies, constitute the entire agreement between you and GlucoWorks regarding your use of our services and supersede all prior agreements and understandings.
